Title: ATTESTATIONS OF POLST OR PAS CIRCUMSTANCES, POST MORTEM

The physician-of-record at the time of a person's death in the State of Florida shall check off one of two checkbox items appearing on the official death certificate. One checkbox option shall be "Attending Physician/Licensed Authority of Record at time of patient's death invokes either P.O.L.S.T. (Physician's Order for Life Sustaining Treatment) or P.A.S. (Physician-assisted Suicide) legal immunity from investigation concerning aforementioned patient's death". Another death certificate checkbox option that may be chosen is "Not Applicable". Only one of the checkbox options may be chosen by the attending physician/legal authority at the time of death. A death certificate completed within the state of Florida must have one box or the other checked by the attending physician/legal authority.
